This is determined by the Care Professionals recommendation. Some Participants Combine Care Packages with Distance Packages in order to benefit from the full spectrum of expertise available.
The 2nd Phase of Care includes education on Self-Care techniques; reassessment of presenting complaints and reduction in care schedule
The 3rd Phase of Care is release from Chronic Care Concerns and Maintenance scheduling
The 4th and final phase is Maintenance. Just like your vehicle, your body and mind require maintenance and this monthly payment allows for 2 additional care appointments in any given month to address an emergency (IE. slip and fall; poor self-care... 'the holidays always produce this effect'; cold or flu related pain; social/emotional trauma)
